Taking Neighborhood Health to Heart (TNH2H) conducted an environmental audit and survey of 950 residents living in five neighborhoods in northeast Denver and northwest Aurora. After larger community presentations and a local conference, the organization has continued to use multiple forms of outreach and education to translate the findings from the research to share and engage with community. In 2009, community members within a sub-committee of TNH2H submitted a pilot grant proposal to use household meetings and small group settings to disseminate study findings and gauge community interest for change. All aspects of organizational infrastructure will be discussed in the context of educational efforts and community building aimed at reducing health disparities. Successes and challenges in building community capacity, while also trying to impact institutional changes will be discussed.
