This presentation will share the Tri-County Health Department experience with activities funded by the Centers for Disease Control and Prevention (CDC) Communities Putting Prevention to Work (CPPW) grant initiative. Activities of this grant provide examples of how a health department can play a leadership role in initiating sustainable policy, system and environmental changes in schools and communities to promote healthy eating and physical activity or obesity prevention. This project can provide ideas for health departments to conduct additional prevention activities that could be funded with resources supplied by the new health reform legislation.
