HIV/AIDS is a disease with a global impact including in the United States. The public health response to it remains strongly influenced by an informed and engaged constituency since the early days of the epidemic. Highly organized advocacy at the patient, service provider, community and legislative levels resulted in significant resources allocated for HIV prevention, care and research initiatives nationally and globally. These programs, especially the Ryan White CARE Act, initially enacted by Congress in 1990, continue to serve as a lifeline for people living with HIV/AIDS in the USA who have no other means of support. An overview and analysis of health reform will be made, as there are potentially positive outcomes and significant quality and access challenges for people living with HIV/AIDS that may occur as a result of health reform policies.
