5066.0 Implementation of High Performing Well Child Care: Coordinating Quality Improvement Efforts between a Large Managed Care Organization, a Community Safety-Net Clinic, a Children's Hospital, and a State Organization

With this session we will provide a real-world example of the implementation of a quality improvement project at a large managed care organization (MCO) and a community safety-net organization. Utilizing findings from dissemination science we will demonstrate how we have begun implementation and dissemination of a system of high performing well child care across the MCO, the barriers uncovered and modifications made to translate this system to the community safety-net organization concurrently with the implementation of a state-wide initiative to increase utilization of the Ages and Stages Questionnaire (ASQ) at all well child care visits. We will also show how the development of this partnership intended to translate a program from the MCO to the safety net clinic resulted in translation of information from the safety net to the MCO by uncovering an immediate need that can be addressed at both institutions. Finally, we will present developing processes to improve standard of care in and ensure consistency of procedures and outcomes measures through collaboration of both institutions and the Children's Hospital in collaboration with the state organization. We hope that the presentation of the real world experiences of change implementation will entice many key stakeholders and facilitate further bidirectional relationship between the organizations for further learning and dissemination of this and other quality improvement processes.
Session Objectives: 1. Define concepts of implementation and dissemination science. 2. Discuss the components of 21st Century well child care. 3. Identify the limitations of internet technology in an urban population. 4. Formulate how multiple institutions form collaboratives in response to shared issues.
