235236 Thailand – Comprehensive approach improves access to HIV/AIDS treatment for HIV-infected children belonging to discriminated ethnic minorities

BACKGROUND: Chiangrai, in the northernmost province of Thailand bordering Myanmar and Lao PDR, has a very diverse population.12.5% of the total population (1.2 million) belongs to 20 ethnic groups which, due to their language and customs, are discriminated against and set aside by mainstream Thai culture. These minority groups are not able to access treatment or information on the health care system or HIV/AIDS, including information on care and treatment that is available to them. HIV-positive ethnic children and families find it difficult to access services critical to their survival and quality of life. THE PROJECT: Plan International, a child-centered humanitarian organization, has implemented a three-year comprehensive HIV/AIDS care and support project for children infected with HIV/AIDS in cooperation with Chiangrai Provincial Hospital since 2008. This project focuses on Chiangrai's minority groups and includes strengthening the pediatric ARV clinic at the hospital, training government and NGO frontline health workers on the treatment and care of HIV-infected children and carrying out home visits to persons living with HIV/AIDS. Activities include counseling, day care, home visits, psychosocial camps and transportation support for children and their families. RESULTS: To date, the project has provided ARV treatment to 562 HIV+ children, attaining treatment adherence greater than 95%. LESSONS LEARNED: Coordinating project activities with the government institutions and community groups facilitated the treatment and care of large numbers of HIV-infected children. Equally important, psychosocial support and economic assistance were provided to affected children and their families to improve their quality of life.
