237552 Overview of the County Health Rankings

This session will describe the population health model upon which the County Health Rankings is built and the Rankings measures and methods. The Mobilizing Action Toward Community Health (MATCH) model will be presented and linkages with community health improvement planning will be articulated.

I have practiced in numerous public health settings at CDC, Michigan and Wisconsin State Health Departments.
