243649 Pacific Innovation Collaborative (PIC) Dashboard: Using a regional Health Information Exchange to monitor patient health in Hawaii and Washington statesWednesday, November 2, 2011: 10:50 AM
The Pacific Innovation Collaborative (PIC) Health Information Technology Network is a collaboration between the Association of Asian Pacific Community Health Organizations (AAPCHO), eight Federally Qualified Health Centers (FQHCs), and two health plans in Hawaii and Washington states. The intent of this network is to utilize the technology established at FQHCs to improve the quality and cost-effectiveness of healthcare to the low-income, high risk and often underserved patient populations they serve. The Pacific Innovation Collaborative (PIC) Dashboard is a user-friendly summary report of patient data for health centers and includes the following measures: childhood immunization, diabetes, primary care visits, ER visits, and well-child visits. These measures are tracked through a combination of claims, practice management system, and electronic health record data. A regional Health Information Exchange (HIE), such as the PIC Dashboard, can be used by managerial and quality assurance staff at FQHCs in different states to monitor, analyze, and improve patient health outcomes.
