Social determinants of health can be mitigated through an informed and participatory community approach to health and wellness. This presentation will explain how federally qualified health centers with vision services positively impact social conditions in a community. Federally qualified health centers offer an array of services designed to meet the needs of communities through addressing disparities in health. Access to vision services has a direct impact on social issues such as improved outcomes in learning, prevention of vision loss, and safer living environments. Health centers are uniquely positioned to offer vision services as a component of comprehensive primary and preventive care services, resulting in improved health outcomes and community wellness.
