This presentation will provide a macro perspective of the NYC Obesity Prevention and Management Consortium by presenting details on how the Community Health Care Association of New York State structured the initiative for health centers in NYC. The presenter will review information on the Consortium's target goals; key partners utilized; strategies to implementing the different arms of the program; overall best practices; and next steps. Data outcomes collected over the span of the Consortium's three-year span will also be shared and explored.
Learning Objectives: Develop strategies for engaging community based organizations as partners to combat childhood obesity.
Identify processes to increase documentation of Body Mass Index (BMI) and Healthy Weight Classification within an Electronic Medical Record.
