This presentation will provide a micro perspective of Morris Heights Health Center; a NYC community health center that participated in the NYC Obesity Prevention and Management Consortium for two years. The presentation will focus on the health center's growth as a participant by exploring the health center's Consortium aim statement, health center team development, data collection process, programs developed to specifically address childhood obesity in their community and other systems changes that resulted from participating in this initiative.
