Unity Health Care began providing health care inside the jail in October 2006 and thus offered continuity of care for from jail to the outside through Unity's established network of community health centers and homeless health care sites. This is a model of continuity of care with a direct connection between the jail and the community. Subsequently Unity opened a Re-entry Health Center that specifically services returning DC citizens from prison and assists with their social service and health care needs.
