As reentry programs emerge throughout the country to address the growing numbers of individuals released to the community with chronic medical conditions, few have designed robust evaluations of their programs. This presentation will discuss some key components of evaluating transitional clinics using community based participatory research methods and particular strengths and pitfalls of this approach.

As such, I was responsible for the design, implementation, analysis of the Transitions Clinic in San Francisco, CA. The evaluation of the Transitions clinic will be a basis for discussion about developing criteria by which to evaluate the work of different transition clinics.
