Presenting author's disclosure statement:
Qualified on the content I am responsible for because: I am qualified to present because I represent the Public Health Foundation (PHF) in communications with CDC, national partner organizations, and grantees. My office triages technical assistance (TA) requests from grantees via CDC, assesses specific TA needs in the realms of performance management (PM) and quality improvement (QI), and coordinates provision of on-site and remote TA in response to those needs, ensuring those services are consistent with evidence-based best practice. I also coordinates the development and distribution of cross-cutting QI tools for NPHII grantees. I agree to comply with the American Public Health Association Conflict of Interest and Commercial Support Guidelines, and to disclose to the participants any off-label or experimental uses of a commercial product or service discussed in my presentation.
