253861 A State Experience in Developing Practice-Based Evidence

This presentation will describe tools capable of identifying needed nutritional interventions for improving community health by addressing a social determinant of health. In addition, the impact of these interventions will be discussed. Examples are drawn from a state's public health experiences.

Learning Objectives:
1. Describe tools used to identify food desserts and the need for supermarkets and farmers markets to meet daily dietary recommendations for fruits and vegetables. 2. Describe available tools that provide standard evaluation methods and measures capable of providing practice-based evidence.
