256059 Strategic Mapping in State Primary Care EffortsMonday, October 29, 2012
Primary care work in the states continues to be an essential part of access to care for underserved and rural areas. Disparities exist throughout this country and states are taking several steps to address population health with federal and local efforts. In an effort to support these continuing efforts, the Association of State and Territorial Health Officials (ASTHO) convened several multi-level and multi-sector partners to strategically map out support for state and territorial efforts to improve state and territorial primary care efforts. The result is a strategic map that builds out the cross-cutting priority of ASTHO into an instrument that can:
Drive all of ASTHO's work in primary care Ensure alignment with key partners and with several levels Provide guidance for the state public health infrastructure The mapping meeting included a substantive dialogue and sharing of ideas between top subject matter experts throughout the country, examined the current role of state and territories in primary care efforts, and how ASTHO can best support these efforts. This presentation will be a unique look at how strategic mapping can support primary care efforts in the states and territories and provide direction for support of this vital area.
