It is estimated that twenty-five percent of hospital admission are due to a preventable adverse events relating to a medication. Ensuring medication safety among community dwelling elderly with low health literacy and complex medical needs improves patient care. This presentation reports on the planning, implementation, and process evaluation of a team-based medication safety program that addresses safe medication use among community dwelling elderly patients. Lessons learned through implementing a team-based medication safety program, which has developed through a partnership forged between a college of pharmacy and Adult Day Services Network of Alameda County, will be highlighted. The presentation reports on practice-based strategies that illuminate the role of individual health care providers in a team-based care program that serves a complex patient population with low health literacy. A framework will be described to assist in creating a program that maximizes the mutual benefit of each profession to ensure safe medication use.
