Overview

Two nurse practitioner faculty members will describe a nurse managed health center in San Francisco: Glide Health Services (GHS). It is anticipated that the GHS “story” will provide health care professionals working with medically-underserved individuals in urban clinics with a model of care and specific approaches that will help them to sustain partner-supported, community-based clinics.

The Story in Brief

In partnership, Glide Memorial United Methodist Church, the University of California San Francisco (UCSF) School of Nursing and Saint Francis Memorial Hospital, GHS has offered a nursing-focused primary care, mental health, substance abuse,complementary care and wellness services to San Francisco's homeless and medically-underserved residents since 1997. GHS has been well integrated into the safety net of public and private agencies focused on caring for this population, which includes being a member of the San Francisco Community Clinic Consortium. These partnerships have helped GHS to become an established Federally-Qualified Health Center and federally-supported Health Care for the Homeless provider site. Additionally,GHS provides pre-licensure and advanced practice nursing students, nurse practitioner residents and other health professional students with clinical training opportunities.

GHS providers are addressing access to care concerns, and improving processes and outcomes of care through a comprehensive continuous quality improvement program and service learning initiatives. Furthermore, our partnership with the Institute of Nursing Centers and Alliance of Chicago through the Agency for Healthcare Research and Quality funding has given the support and technical assistance to achieve “meaningful use” of data for performance improvement and improved billing.
