The Agency for Healthcare Research and Quality (AHRQ) developed three videos for primary care clinicians and clinical teams at varying stages in their self-management support (SMS) awareness and implementation. The videos were recorded at diverse clinical settings including primary care practices and feature clinicians who have successfully implemented SMS. The videos illuminate SMS concepts and demonstrate tangible, actionable approaches to integrating SMS into practice, introduce the What, Why and How of SMS, and encourage viewers to examine a library of resources related to each topic. The clips will be available publicly on the AHRQ website (http://www.ahrq.gov/) in spring 2012.
