The purpose of the County Health Rankings is to present data in a way that serves as a call to action. This session will describe how communities are using this information to create multi-sector partnerships that are focusing on policy and systems change to address the multiple factors that influence health.

Qualified on the content I am responsible for because: I have been the Deputy Director for the County Health Roadmaps since April, 2011, and served as the Community Engagement Lead for the County Health Rankings prior to that since September, 2009. Through this work I have worked closely with communities throughout the nation. Prior to my work with the Rankings project, I was a local health official in Wisconsin with 21+ years of local public health advocacy work.
