The Project HOPE, Habits for Life program, is a mobile healthcare screening program with educational and telemedicine components integrated with community organizations with the goal of improving community wellness. Mobile technologies afford a means of increasing access to healthcare screening in underserved areas where they are unavailable. The use of mobile health clinics, in the Habits for Life program, is being integrated into the concept of preventative care at the community level.

Comprehensive health screening is a first step toward improving the health of a community. The concept of widespread community screening to identify risk, and give personal information on which to act, is one strategy that can play an important role in driving individuals to seek preventive or preemptive care before they are in a crisis. Results empower participants to make changes to improve their health status and are an opportunity to motivate people to address their health. Results provide agencies and healthcare provider organizations the tools they need to assess the overall health of the community.

The Habits for Life program is linking supportive services within the healthcare community of hospitals, clinic systems and government organizations through collaborative partnerships and supports the patient-centered medical home model through these relationships.

Our presentation will discuss results for the New Mexico program and strategies used to build health system capacity with our partner organizations.
