Faith and community leaders are trusted messengers in their community and can serve as strong partners in the reduction of health disparities, specifically amongst vulnerable and hard to reach populations. This session will provide case studies and information on the ongoing and diverse public health partnerships that the Office of the Minority Health, Centers for Disease Control, National Vaccine Program Office, and the Center for Faith-Based and Neighborhood Partnerships have with faith and community leaders. In particular, this session will focus on the work done on immunization, H1N1, and seasonal flu.

Describe the unique roles that faith and community leaders play in public health outreach and the reduction of health disparities. Describe the best practices of innovative faith and health partnerships relative to elimination of health disparities.

Qualified on the content I am responsible for because: I have been working with a multiple groups of faith and community stakeholders focusing on health disparity reduction, health equity, and primary care access and prevention. In particular, I have worked to educate faith and community groups on H1N1 and seasonal flu to increase immunization rates in vulnerable and hard to reach communities.
