5139.0 Community Partnerships in Maternal and Child Health: Community-Based Perinatal CareWednesday, October 31, 2012: 10:30 AM - 12:00 PM

This session showcases examples of how community partnerships enhance understanding, implementation, and evaluation of perinatal and interconception care programs. Presenters will describe how doulas and mothers utilize Concept Mapping to identify factors impacting perinatal health, and how community-based doula programs can improve health outcomes while demonstrating cost savings. Additional presenters will describe how community partnerships can be utilized to implement and evaluate culturally competent programs, and engage in a Learning Community to enhance interconception care.
Session Objectives: 1. Compare methods and approaches to improving community based perinatal care (including learning communities and concept mapping).
2. Identify measureable outcomes in perinatal and interconception health programs.
3. Discuss factors and issues impacting the outcomes of community based programs, as examined by presenters.
