This session addresses many issues related to the management of chronic diseases/conditions among the elderly. These include: correlates of completion for the chronic disease self-management program and Tomando Control de su Salud, feasibility of physical telerehabilitation in seniors with mobility limitation, outcomes of the diabetes self-management program offered in South Florida through the healthy aging regional collaborative, factors associated with CDSMP completion in a Spanish speaking population in Puerto Rico, and a Healthier Living Program for Seniors (HELPS).
Session Objectives: 1. Identify correlates of completion for the Chronic Disease Self-Management Program and Tomando Control de su Salud. 2. Identify three different tests to measure functional status of patients with limited mobility. 3. Compare the results of this study with other diabetes self-management programs. 4. Assess participants’demographic and health characteristics associated with completion of a chronic disease self-management program (CDSMP) in Puerto Rico.
