Integrating population health into delivery system reform

The transition of the US healthcare delivery system from a payment model based on volume to one based on value offers the prospect of creating a more sustainable payment model that rewards improvements in population health. This session will first provide an overview of current payment reform initiatives at the federal and state level which are intended to transform the care system and drive Triple Aim outcomes. It concludes with an assessment of the opportunities and threats created by the challenges of trying to integrate population health into delivery system reform
