Social Determinants of Health: Breaking barriers at the State level

States play a vital role in the successful implementation of Federal health policy. This presenter will describe how states and communities in New England are putting into practice key Federal health initiatives, including Healthy People 2020,by tackling the root causes of health inequities and disparities.
