Closing Gaps in Chronic Disease

This session explores strategies to close gaps in chronic disease disparities. The topics discussed include disease self-management, engaging physicians as advocates for health policy, and the role of community organizations to improve health.
Session Objectives: Demonstrate the effectiveness of health engagement management strategies to close clinical gaps in chronic disease populations.
