Created by the Affordable Care Act, the Community Transformation Grants (CTG) program supports States and communities that tackle the root causes of poor health so Americans can lead healthier, more productive lives. In this session, invited program grantees will discuss their efforts to date, their success, and their lessons learned.
Session Objectives: Formulate a better understanding of successful strategies that are being implemented at the community level with Community Transformation Grant funds. Formulate an awareness of strategies that could be transferred to other communities.
