Comparing two data collection systems for health information exchange with public health and clinicians

IN 2012 Prince George's County Maryland has established a program to share health information among public health, community clincians and community based organizations. The system they have used to mantain data and support the exchange will be described and the elemental functionality described. In 2002 the Center for Healthy Communties in Montgomery County Ohio began construction of a health information exchange based on the Continuity of Care Record Standard (ASTM). This exchange product has been used to connect to the Nationwide Health Information Network and provides standards based exchange. Presenters for this session will compare and contrast the two systems, lessons learned and methods they have explored for collaboration. Exchange partners will be highlighted, the strategies for community based information sharing, patient engagement and implications for public health policy will be explored.
