Access to healthcare services using a nurse navigation/education model
Statistics, both nationally and locally, show that men access healthcare roughly 10% less often than women. When we combine this with the fact that the healthcare reimbursement model is changing, we realize that we need a new model where men are more engaged in their healthcare.
Our new model establishes a nurse navigator focused on 1) education and motivation for men through corporate and community outreach 2) removing barriers when accessing healthcare and 3) physician education in order to have more effective conversations with men. This model integrates a variety of healthcare services from behavior to wellness. It is also being extended to community based programs through the State of Nebraska in order to create a continuum of care.
Methods such as a one-call-for-all phone number, materials developed specifically for men, streamlining access to providers/services/education and motivational interviewing in both a group and one-on-one setting are all being implemented in order to promote better compliance and generate better outcomes.
Our program is currently being implemented and we will have initial observations and findings by July 2013. Findings that demonstrate an improvement of 1) primary care visits 2) visit compliance 3) new patients seeking ACH and 4) improved health status.
Our ability to engage men in preventative health behaviors will improve the quality of men's lives and allow healthcare systems to better manage their resources.

Qualified on the content I am responsible for because: I have been responsible for healthcare operations at Alegent Creighton Health for over 20 years. My responsibilities have been to develop and implement new business models to meet evey changing healthcare policies. I have worked with physicians, administration and community leaders to implement new programs such as quick care clinics, back and spine centers, new acute care facilities outpatient rehabilitation centers and residential treatment facilities to name just a few.
