Massachusetts chipra medical home project: Promoting patient-centered pediatric care through quality improvement

The Massachusetts CHIPRA Medical Home Initiative involves 13 pediatric practices across the state in a 29-month quality improvement learning collaborative. NICHQ (the National Initiative for Children's Healthcare Quality) is one of several partners to lead the project; others include Massachusetts Health and Human Services, Children's Hospital Boston, Massachusetts Health Quality Partners and University of Massachusetts Medical School. We are working with practices across Massachusetts to facilitate their transition to patient-centered medical homes. This includes adopting methods and tools to address the comprehensive care coordination needs of families. The public health significance of the pediatric medical home model is the shift to include all children, not just children with special health care needs. This modern approach cultivates the partnership between patient, families, providers and community supports as a model to create accessible, family-centered, continuous, comprehensive, coordinated, compassionate and culturally effective care for all children. Teams are testing the reliability and usability of 24 pediatric healthcare quality measures. To date, teams have shown measureable improvements in comprehensive well care, developmental health, mental and oral health, clinical outcomes, patient safety, care planning, transition to adulthood, efficiency, and medical home transformation. In this session, we will share results and lessons learned, discuss innovative and promising models, and provide recommendations for future QI efforts especially as they apply to similar topic areas or settings. This is session 3 of 4 in a panel titled, “Applying quality improvement in public health systems for maternal and child health.”
