Qualified on the content I am responsible for because: I have planned for and administered the integration of the CHOICES program for the past 2 years and have developed and implemented several other programs in tertiary care and ambulatory settings throughout my 30 year career in health care. I have presented at and attended several grant meetings and work groups at the CDC to share our implementation efforts with grantors and colleagues as well as participating in evaluation efforts.
