An evaluation of the chronic care model, national prevention strategy and healthy people 2020 goals to reduce health disparities among older americans thru the patient protection and affordable care act

The Chronic Care Model (CCM) promotes effective change and evidenced-based clinical and quality improvement in healthcare. The National Prevention Strategy's (NPS) goal is to increase the number of Americans who are healthy at every stage of life. NPS concentrates on community environments, preventive services, empowered people, and the elimination of health disparities. Healthy People 2020 (HP2020) goals to achieve health equity, eliminate disparities and create social and physical environments that promote and improve good health for all are foundations for a more equitable society. Our study's objective was to evaluate how the CCM, NPS and HP2020 thru the Patient Protection and Affordable Care Act (PPACA) would reduce disparities in care of older Americans by increasing the percentage of older adults with quality clinical preventive care and services. Our evaluation emphasizes the combination of the CCM, NPS and HP2020, which advances the health of older Americans by increasing the percentage with care coordination and active follow-up to ensure the best healthcare outcomes thru provisions of the PPACA. The CCM, NPS and HP2020 targets of 10% improvement in prevention and reduction of chronic diseases for older Americans would have the potential to enhance quality of healthcare for older adults. After mapping PPACA provisions, we conclude that high-risk pools for pre-existing conditions and insurance rescission bans have the potential to reduce disparities in the quality of care and services for older Americans. Furthermore, the PPACA has the power to strengthen this triangulation regarding the fortification of equity and protecting our most vulnerable populations.
