Biographical Sketch: Alison O. Jordan, CPPB, LCSW is the Executive Director for Transitional Health Care Coordination in the NYC Department of Health and Mental Hygiene and coordinates medical discharge planning programs for those released from City jails, implementing a national model for Transitional Care Services persons leaving City jails and Principal Investigator for a HRSA/SPNS Jails Initiative, the largest evaluation of jail releasees. She has contributed to correctional public health as an author, panelist and contributor.
