Biographical Sketch: I am currently pursuing a doctorate degree in Social Welfare at University of Hawai`i at Mānoa. I have been involved with the Hawai`i Healthy Aging Partnership (HHAP) to effectively embed evidence-based programs such as Stanford's Chronic Disease Self-Managment Program in community-based organizations, and serving as an evaluator for more than 7 years. During this time, I have provided leadership to HHAP members with project dissemination, through manuscripts and local and national conference presentations.
