Advancing the dissemination and implementation of evidence-based practices in Community Health Centers: Research of the Cancer Prevention and Control Research Network (CPCRN)

CPCRN is a national network funded by the CDC and NCI that includes academic, public health, and community partners who work together to reduce the burden of cancer, especially among those who are disproportionately affected. The session includes five presentations that highlight three studies conducted by the CPCRN’s Federal Qualified Health Center Workgroup over the past two years to advance dissemination and implementation of evidence-based practices (EBPs) in Community Health Centers (CHCs). Complementing each other in terms of topics and methodologies, they contribute a broad and deep understanding of the implementation of EBPs in CHCs on their journeys to becoming Patient-Centered Medical Homes (PCMHs).
Session Objectives: Explain four modifiable evidence-based behaviors for improving care.
