TRACE-CORE: lessons for contemporary longitudinal health outcomes research studies. Transitions, Risks and Actions in Coronary Events – Center for Outcomes Research and Education (TRACE-CORE) is an ongoing study that has enrolled 2300 patients with acute coronary syndrome from hospitals in Massachusetts and Georgia. It is following them, with intensive data-capture covering multiple domains, for one year. TRACE-CORE seeks to understand relationships among transitional and post-acute care, patient cognitive status and quality of life and other non-traditional outcomes, such as hospital readmissions.
