This presentation accomplishes two objectives. (1) Demonstrates the Metrics for Healthy Communities online tool which is now available to support increased use of data and metrics among public health and community development practitioners. The tool contains sample logic models, frequently used measures, and direct links to data available at different geographic levels. (2) Describes how this resource can help guide and measure the impact of collaborative health improvement initiatives, and what implications the tool and its underlying research have for developing new measures and frameworks to support health in all policies.
