Survivorship care plans association with patient-provider communication, receipt of regular check-up, and having a regular source of care: A population-based analysis

Jennifer M. Jabson, PhD, MPH, Department of Public Health, University of Tennessee, Knoxville, TN
Background: Survivorship cares plans (SCP), including treatment summaries (TS) and follow-up care instructions (FCI), are recommended for all cancer survivors. SCPs are intended to promote patient-provider communication, and ensure survivors receipt of follow-up and regular primary care.  Existing findings concerning SCP are mixed, and there is limited published evidence regarding the relationship between SCP and patient-provider communication, receipt of recent check-up, and having a regular primary care doctor.

Methods: Data are from cancer survivors (N=464) in the Cycle 2, 2013, Health Information National Trends Survey (HINTS).  Multivariate logistic and linear regressions tested for associations between SCP, FCI, and TS and having a regular provider, patient-provider communication, and time since most recent check-up.

Results:  In this sample, 79% received FCI, 34% received TS, and 38% received SCP (FCI+TS).  Those who received FCI had less time since recent check-up (b = -.18, p = .03) than those without FCI.  Survivors who received TS had better patient-provider communication (b = -1.16, p = .02) than those without TS.  Associations between SCP and time since recent check-up and patient-provider communication trended toward but did not achieve statistical significance.

Conclusion: SCP and its individual components may be important for patient-provider communication and getting regular check-ups, both of which are relevant to cancer survivor health and disease prevention.

Learning Areas:

Chronic disease management and prevention
Epidemiology
Social and behavioral sciences

Learning Objectives:
Describe the distribution of survivorship care plans received by cancer survivors at the population level. Discuss the associations between receipt of survivorship care plans, and its components, with cancer survivors’ patient-provider communication, receipt of recent check-up, and having a regular primary care doctor.

I conduct research concerning survivorship care planning and survivors’ health and access to care after cancer. My research has been published in high ranking peer-reviewed journals and I have completed research projects and received funding for research conducted with cancer survivors.
