Communities around the country are learning to use collaborative and partnership skills to solve complex issues together.  The session will focus on key elements of successful partnerships for advancing population health – pulled from 81 communities across the country in the evaluation of the first three cohorts of the National Leadership Academy for the Public’s Health.  Center for Health Leadership and Practice (CHLP) uses an innovative approach to leadership development that supports teams composed of members from multiple sectors to build work that is responsive to local needs for improved community health.  Through these community stories, the session will also highlight the CHLP Leadership Model, competencies, program phases, elements and methodologies.
