Lack of basic surgical care affects over two billion people around the world, resulting in catastrophic economic, moral, and social consequences. Essential surgical services address surgical diseases that are largely neglected, including obstructed labor, congenital diseases, cataracts, hernias, and basic surgical care for injuries, emergencies, and consequences of infectious diseases. These conditions continue to represent a public health challenge that disproportionately impacts women, children, and the world’s most vulnerable populations.  Global surgical care and anaesthesia disparities have a far-reaching impact, affecting millions of people worldwide, across all socioeconomic and demographic strata. In light of a number of upcoming global milestones, including the anticipated introduction of a World Health Assembly resolution recognizing surgical care and anaesthesia as an integral part of universal health coverage, the launch of the Lancet Commission on Global Surgery, and the pending introduction of the United Nation's sustainable development goals which provide a post-2015 global development framework, there has never been a more opportune time to raise the international profile of surgical care. This presentation will include an analysis of the current state of global health and surgical care, a review of current advocacy efforts and a discussion of the integral role of surgery and anaesthesia as part of universal health coverage and the post-2015 sustainable development agenda. The presentation will also include a review of efforts within the global surgery community to advance targets and metrics to support the integration of safe, essential surgical care as part of national health plans. As the world looks towards the age of sustainable development, the global public health community must not miss this critical window of opportunity to advocate for the provision of quality, safe, surgical care for the global population.
