The care coordination field is inconsistent with its definitions, operations, and documentation of effective care coordination service delivery.  The lack of standardization contributes to the duplication of services and gaps in client/patient connections to needed services.  This presentation will discuss the need for research on the how standardization of care coordination services can lead to quality improvement.
