Presentation will highlight examples of a nationally-led collaborative focused on the integration of public health and primary care. The ASTHO-supported Integration Forum connecting Primary Care and Public Health is a partnership of more than 50 organizations and over 100 individual partners seeking to inform, align, and support the implementation of integrated efforts that improve population health and lower health costs. Active partners include national, state, and local primary care and public health leaders. Over 50 national organizations represent public health at the state and local level.

Qualified on the content I am responsible for because: I have been a public health leader for over 25 years at the state and national level. As the ASTHO Chief of Health Promotion and Disease Prevention, I have been a staff lead on the development and implementation of the ASTHO Primary Care and Public Health Collaborative. This has included: development of strategic maps with national partners, facilitation of bi-monthly calls with 60+ organizations, and Co-Chair of the Collaborative’s Successes and Measurement Workgroup.
