Plan4Health: Linking Public Health and Planning to Build Improvements in Health

This session will focus attention to the work being accomplished by Plan4Health Coalitions that were funded by the Centers for Disease Control and Prevention through the APHA and the American Planning Association in early 2015. This joint effort brings together professionals from both the Public Health and Planning communities, together with community partners, to improve physical environments in high need areas making healthier choices, easier choices for community members. Presenters will discuss coalition development, establishment of community priorities, implementation of environmental improvements and methods to measure improvements in health outcomes as a result of the projects implementation.
Session Objectives: Discuss the Plan4Health goals and implementation plans Discuss the process of community coalition development, goal setting and evaluation plans Discuss how the Plan4Health projects can be replicated, within communities, throughout the nation
