Nursing Home Care and Care Transition Patterns

Session Objectives: Describe how increasing proportions of short-stay residents under Medicare skilled nursing facility (SNF) coverage in nursing homes relates to changes in long-stay nursing home residents' independence in performing activities of daily living (ADLs). Analyze the factors associated with utilizing a long-term care(LTC) institution upon hospital discharge, in older populations. Describe how the receipt of palliative care consults affects acute care use for persons dying in nursing homes with dementia. Analyze if the presence of hospital-based skilled nursing facilities in hospitals leads to lower readmission rates.
