Innovative quality management strategies to improve tracking of health outcomes for Ryan White Case Management

Issue: A lack of continuous monitoring and feedback regarding health outcomes can often produce significant gaps in data quality and limit case management organizations' ability to evaluate the relationship between case management and client health.
Description: As the lead agent for case management in the Chicago EMA, the AIDS Foundation of Chicago (AFC) is responsible for monitoring the quality of services provided by subcontractors, however, the reporting database’s limitations for pulling performance reports has significantly prevented agencies from monitoring their own performance. As such, AFC has implemented several initiatives to specifically address these issues as well as provide feedback on the quality of provided services. Firstly, a dashboard was created to monitor core case management performance measures during the grant year. These 'dashboard' reports were distributed on a quarterly basis to enable case managers to visualize and monitor changes in performance across their caseloads. Secondly, a sample of 10% of case managed clients at each agency was queried at random for an in-depth qualitative evaluation to determine whether clients’ perceived barriers and health concerns were addressed by case managers’ service provision.
Lessons Learned: Implementation of dashboards has not only allowed agencies to identify and isolate specific gaps in services providing opportunities for targeted interventions but also pinpointed specific areas requiring technical assistance.
Recommendations: Future interventions on other means of monitoring the quality of provided services would be important to provide insight into improving health outcomes for case managed clients.
