Leveraging Innovative Grants to Develop, Evaluate and Sustain Evidence-Informed Interventions: Lessons from Rikers Island and Puerto Rico Special Projects of National Significance Initiatives

Currently NYC Correctional Health Services (CHS) is implementing and evaluating two interventions that are the outgrowth of the SPNS Jail Linkages Initiative’s Transitional Care Coordination model to: 1) improve provider cultural competency around working with justice-involved HIV patients of Puerto Rican origin and 2) deliver similar services in Puerto Rico. The enhanced interventions and concurrent evaluations were made possible through three federal HRSA Special Programs of National Significance (SPNS) grants which promote developing and evaluating program enhancements that can be integrated and sustained. Disseminating findings and publishing evaluation outcomes across multiple sites allows for building evidence-informed interventions that may be replicated.
This presentation will include a chronology of CHS experience with SPNS grants including the purpose of applying for each grant, evaluation findings to date, and the benefits gained, including intervention sustainability and growth. The presentation will end with a discussion of SPNS and other funding opportunities that may be of interest to community and other providers to evaluate and disseminate findings from their programs.
