This short, animated video introduces and describes Prevention Institute's Community-Centered Health Homes model, a bridge between healthcare organizations and the communities they serve. The three-minute video explains the positive impact healthcare organizations can have when they take on the non-medical factors that impact their patients’ lives, like food insecurity, poor housing conditions, community violence, and others. In it, a fictional Dr. Lau sees patient after patient suffering from asthma attacks that are being triggered by community conditions like mold in patients’ homes and high levels of air pollution. Like so many real-life healthcare practitioners, she realizes clinical inventions alone can't get her patients the health outcomes they want without complementary community interventions, and she decides to take action.
