The Navigation Task Force (NTF), a pivotal subsystem of SDIT, provides guidance and support for participants enrolled in diabetes prevention and management programs to find and utilize local social and community resources. Navigator Teams consist of intern professionals in social work and nursing who collaborate to develop personalized ecologically sensitive navigation care management plans that: Assists, Refers and Follows participants. </p>\n<p>Methods </p>\n<p>SE is applied to develop solutions for the integration of navigation services within clinical and community resources. A systems architecture and plan were developed and implemented in year 1 consisting of 6 subsystems of which navigation is central. A systems audit and risk assessment were conducted in year 2 to identify unforeseen and potential future risks within and across the multiple subsystems that could affect the mission of NTF. Arising from year 2 risk mitigation considerations, a Navigation Guidebook was developed as a training and fidelity-assurance instrument to prepare and provide guidelines for Navigators in their role in navigating participants through integrative social and healthcare services.</p>\n<p>Conclusion</p>\n<p>Using SE to create a guidebook was critical to the structure of the organization for navigation services.
