Providing equitable access to resources and services that optimize the health and well-being across the life course is an important aspect of healthy aging. Achieving health equity involves adopting new policies and changing systems so that everyone has the opportunity to live the healthiest life possible. The Association for State and Territorial Health Officials (ASTHO) partnered with Trust for America’s Health in the development of the Age-Friendly Public Health Systems 6Cs Training and Implementation Guide (6Cs Guide). The 6Cs Guide is a public health tool designed to build capacity and expertise in age-friendly public health practice. ASTHO was instrumental in developing the operationalizing equity supplement for the 6Cs Guide. Strategies and approaches to advance health equity are included in many of guide’s six modules as well. This session will provide an overview of the strategies to operationalize equity in healthy aging practice and how these approaches address the social determinants of health.
