Engaging healthcare professionals to increase referrals to lupus self-management programs through tailored tools and strategic partnerships

The Lupus Foundation of America (LFA) is leading efforts to increase access to evidence-based lupus self-management (SM) programs through healthcare professional (HCP) engagement and systems-level innovation. The LFA has developed a suite of referral tools—including a print-based SM guide and tailored marketing materials—to support HCPs in connecting patients with programs in English and in Spanish to increase lupus SM behaviors and improve health outcomes. Informed by formative research with HCPs, health literacy experts, and people with lupus, LFA identified barriers to SM referrals including limited clinical time, lack of interoperable electronic health records, and gaps in provider knowledge.

To address these barriers, the LFA piloted HCP-focused referral packets, EHR referral mechanisms, and targeted materials such as appointment cards and clinic breakroom displays. These tools were tested two Area Health Education Center and deployed to HCPs across the country through its partner network to optimize usability and integration into clinical settings. In tandem, HCP education was deployed nationally to enhance provider knowledge of lupus symptoms, treatment, patient support programs, and SM referrals.

This multifaceted approach advances health equity by ensuring lupus care and support services are accessible, user-centered, and aligned with the needs of both patients and providers. LFA’s work highlights the importance of provider engagement, co-design, and tailored dissemination strategies in scaling public health interventions for chronic disease management.
